If you would like to investigate Lab Work and Outpatient Surgery options on your own, Capital BlueCross offers that option.
You discover you need
a medical treatment
Call a Care Coordinator, explain what you need and if you have a preferred provider.
Your Care Coordinator will conduct research on what you need, where you prefer to receive your care and what that care will cost at a number of facilities in your area. Your Care Coordinator can provide you with a cost comparison report.
If you receive care such as a procedure or surgery at one of the lower-cost facilities, you will receive a cash reward (taxable, included in your pay) of 20% of the difference in the cost of the higher cost facility, up to a maximum of$1,000. (Limited to one reward per procedure per year.)
Need to find a doctor or location for a service? Mary was having a difficult time finding an in-network provider and knew that choosing someone in the network would save her money. She contacted a Care Coordinator who found an in-network provider close to her home and even scheduled the appointment for her.

Visit with an in-network provider anytime through your computer, smartphone, or tablet. Doctors can diagnose you, recommend treatment, and prescribe medication. Video visits can be used for non-emergency illnesses, such as sinus infections, flu, pink eye, or strep throat. Or, visit a specialist for counseling for anxiety, grief, depression, trauma, etc. You pay $10 per primary care visit. For specialists, you pay 30% under the Silver Plan (or 25% under the Gold Plan) after you meet the deductible.

The goal of care management is to help you achieve wellness,
stability, and independence by taking an active role in improving your health. If you need complex medical care,
Capital BlueCross works with you, your doctors, and other
health care providers to create a care plan tailored to your

To find an independent lab, log in to your secure account at capbluecross.com. Then choose Find > Doctors, select “Labs and Radiology” from the drop-down menu, check “Lab” in the Specialty section, and select your network. Click “Search” and look for independent clinical laboratories.
To find an ambulatory surgical center, choose Find > Doctors, then select “Hospital” in the Provider field, then check “Ambulatory Surgical Center” in the Specialty section and select your network.
You have a choice when it comes to where you go for lab work and outpatient surgery. If you have lab and surgery services at facilities rather than independent locations, you will pay higher coinsurance.
The Traditional Plan is a grandfathered plan - meaning only those currently enrolled in the Traditional Plan are eligible to re-enroll.

Thomas R. and Laura Ridge Scholarship
Weis Markets offers a slate of benefits to help you meet your current and future financial needs. You have two opportunities to apply for Scholarships for yourself or your dependent: The Weis Markets Scholarship Program and the Thomas R. and Laura Ridge Scholarship.
30 traditional four-year scholarships at $2500 per year
10 non-traditional two- to four-year scholarships at $2500 per year
Download and complete the editable APPLICATION form, fill in the required fields, and follow the directions for submission by mail or email by the application deadline.
Download the SCHOOL OFFICIAL STATEMENT form, fill in the required applicant fields, email to your school guidance counselor or school official for completion. The completed form must be submitted (with required documentation) by the application deadline.
If you (the applicant) are currently employed, download the EMPLOYER REFERENCE LETTER form, complete the fields required of you, and email the form to your supervisor for completion of their portion. Your supervisor will need to submit this form by the application deadline.
Associates who have been employed at least one year as of March 1, 2021 and are in good standing with the company are eligible. Complete the steps below and email your documents to HR@weismarkets.com by March 31, 2021.

The Pennsylvania Food Merchants Association is pleased to announce that Thomas R. and Laura Ridge Scholarship money is available for the 2021-22 school year.
The scholarships are named in honor of Thomas R. and Laura Ridge, the parents of former Pennsylvania Governor Tom Ridge. Thomas R. Ridge was a member of the Pennsylvania food industry for 25 years.

His wife, Laura, was a strong advocate of education for their children Tom, David and Vikki. This scholarship fund was created to honor their memory.

An independent judging panel will select the winners based on academic success, overall excellence of character and potential for community contributions and leadership.
