You discover you need
a medical treatment
or procedure.

Call a Care Coordinator, explain what you need and if you have a preferred provider.

Your Care Coordinator will conduct research on what you need, where you prefer to receive your care and what that care will cost at a number of facilities in your area. Your Care Coordinator can provide you with a cost comparison report.

If you receive care such as a procedure or surgery at one of the lower-cost facilities, you will receive a cash reward (taxable, included in your pay) of 20% of the difference in the cost of the higher cost facility, up to a maximum of$1,000. (Limited to one reward per procedure per year.)

To find an independent lab, log in to your secure account at capbluecross.com. Then choose Find > Doctors, select “Labs and Radiology” from the drop-down menu, check “Lab” in the Specialty section, and select your network. Click “Search” and look for independent clinical laboratories.
To find an ambulatory surgical center, choose Find > Doctors, then select “Hospital” in the Provider field, then check “Ambulatory Surgical Center” in the Specialty section and select your network.
You have a choice when it comes to where you go for lab work and outpatient surgery. If you have lab and surgery services at facilities rather than independent locations, you will pay higher coinsurance.

If you (the applicant) are currently employed, download the EMPLOYER REFERENCE LETTER form, complete the fields required of you, and email the form to your supervisor for completion of their portion. Your supervisor will need to submit this form by the application deadline.

The scholarships are named in honor of Thomas R. and Laura Ridge, the parents of former Pennsylvania Governor Tom Ridge. Thomas R. Ridge was a member of the Pennsylvania food industry for 25 years.

The Pennsylvania Food Association is pleased to announce that
Thomas R. and Laura Ridge Scholarship money is available for the 2022-23 school year.

The scholarships are named in honor of Thomas R. and Laura Ridge, parents of former Pennsylvania Gov. Tom Ridge. Thomas R. Ridge was a member of the Pennsylvania food industry for 25 years. Laura, was a strong advocate of education for their children Tom, David and Vikki.
Applications and all materials must be received via the online application by TUESDAY, MARCH 1, 2022. Applicants will be notified no later than May 31, 2022, via email. It is the applicant’s responsibility to ensure all materials are received prior to the deadline.
An independent judging panel will select winners based on academic success, excellence of character and potential for community contributions and leadership.
